From Katrina Help Info

There seem to be two different forms. One of them is on a white background and has redundant fields (posted at, date, url AND source, date, url). The other is on an orange background and much simpler to use. I'm betting the simple one is the one we should be using (just because that's bureaucracy). I just want to be sure the data I'm entering is going somewhere and helping someone, not just sitting on someone's server. Any insight to this would be great!
